At Spine Muscles Nerves, we accept a wide range of insurance plans. When completing the initial intake, we gather your insurance information. We will copy your insurance card(s) and complete prior authorizations for all procedures, treatments, and medications.

 

If you wish to use medical insurance, important information regarding your health benefits is available directly from your insurance provider. It is your responsibility to understand your coverage and out-of-pocket costs.

When contacting your insurance provider regarding coverage, state that you are calling about Outpatient Health Benefits. Here are some questions to help determine your benefit level:

 

• Is Dr. Robert Gould considered in-network or out-of-network for my plan?

• What are my in-network and out-of-network annual deductible amounts?

• How much of my deductible I met?

• What is my co-pay or co-insurance once I meet the deductible?

• How many visits are allowable per calendar year?

• Do pain management health services require prior authorization?

• What is the effective date of my coverage?
